Summary of Guidelines for Managing Histoplasmosis among People Living with HIV

Histoplasmosis is a frequent fungal opportunistic infection in people living with HIV (PLHIV), associated every year to total of 5% 15% AIDS-related deaths among this population. In 2020, the first global guidelines for diagnosing and managing disseminated histoplasmosis PLHIV was published. This document recommends (1) detection circulating Histoplasma antigens as recommended laboratory assay diagnose PLHIV; (2) use liposomal amphotericin induction therapy severe or moderately disease, followed by maintenance itraconazole 12 months; shorter could be considered if patient clinically stable immune status has improved; (3) antiretroviral initiation soon possible patients without involvement central nervous system; (4) that treatment co-infection tuberculosis (TB), TB should initiated according World Health Organization guidelines. Appropriate health education providers, supportive supervision, policy guidance care are required.
